Coordination of Care

Care coordination is the efficient communication and delivery of care between providers, insurance plans and caregiver, e.g. the coordination between a GP, a hospital and a care home. The care may be delivered through traditional methods or technologically innovative and more efficient ways to meet changing healthcare demands.

At Pearlfect Care, our pharmacist Abidiemi Asher Yusuf can effectively communicate and coordinate care between providers, caregivers and insurance plans when patients start medication and throughout the whole treatment. He has many years of experience in collaborating with hospitals, GP surgeries, healthcare professionals and more for onward referrals.

Yusuf can also collaborate with hospital teams after patient discharge to facilitate a smooth therapist induction, when required. During mid-treatment calls, he will assess medical adherence, side effects, efficacy and address any other questions the patient may have. Additionally, he can coordinate care by reaching out to patients on existing medications at the start of their new treatment and at key intervals throughout, ensuring they receive the best care.
